34-year-old female is one-year postoperatve from a revision rhinoplasty with Dr. Barrera. She had a rhinoplasty in Japan with a silicone implant. She wanted removal of the implant with subsequent dorsal augmentation, tip refinement, and elevation of her columella. Dr. Barrera performed a revision rhinoplasty consisting of a septal extension graft from costal cartilage, a  small spreader graft was fashioned from the patient’s native septal cartilage and secured between the left upper lateral cartilage and lateral crural tension sutures were placed to create new domes and increase rotation, securing the lower lateral cartilages to the septal extension graft.
