Description
49-year-old male presents for initial consultation for rhinoplasty. He had trauma and a broken nose and it was never fixed. He has had a crooked nose since. He has trouble breathing out his nose. A septorhinoplasty and ultrasonic rhinoplasty was performed. The left upper lateral cartilage was sharply divided from the septum on the caudal edge for placement of a spreader graft to open the nasal valve. The spreader was then placed along the septum right medial to the upper lateral cartilages and this was secured to the septum with multiple 5-0 prolene mattress sutures. A septal extension graft was placed and secured to the caudal septum with 3-0 prolene. The patient’s septal and boney hump was identified. The septal portion was removed with a 15 blade scalpel and the bony hump taken down sharply with a Piezo diamond rasp. He is 3 months postop
